1. Purpose
Move Healthcare is committed to protecting the privacy, dignity, and rights of all stakeholders, in line with relevant legislation including the Privacy Act 1988, Australian Privacy Principles (APPs), NDIS Act 2013, Aged Care Act 1997, and NSW Health Records Act 2002.
We collect only the personal and sensitive information necessary to deliver safe, high-quality, and tailored services, such as health records and disability status, ensuring accuracy, security, and confidentiality.
Information is stored securely in Cliniko with two-factor authentication and antivirus protection. Physical records are retained in accordance with NSW retention laws (a minimum of 7 years, or until a minor turns 25), after which they are confidentially destroyed.
2. Use of Personal Information
Personal information is used only to deliver services that meet your needs and legal entitlements. Disclosure will occur only with your consent, or when required or authorised by law, including to:
- Other health or medical practitioners involved in your care
- Employees, contractors, or supervised students
- Relevant government departments, including NDIS and aged care regulators
- Law enforcement agencies or where records are legally required
- Situations involving serious risk to health or safety
3. Giving Consent
Where disclosure of personal information is necessary, written consent will be requested and the reason for disclosure clearly explained. You may choose what information is shared, nominate an advocate, and may refuse or withdraw consent at any time.
When providing consent, you should be confident that:
- Consent is given voluntarily
- You understand what consent means
- You have been informed of this Privacy Policy
- You understand what information is required and why
- You can raise any privacy concerns at any time
Separate written consent will be obtained prior to any audio or visual recordings being collected.
4. Accessing Your Information
You may request access to your personal information at any time. Access will be provided following identity verification, unless Move Healthcare is lawfully required or authorised to withhold access.
5. Data Breach Response
In the event of a suspected or confirmed data breach, immediate steps will be taken to contain the breach and assess associated risks. Where serious harm is likely, affected individuals and the Office of the Australian Information Commissioner (OAIC) will be notified in accordance with the Notifiable Data Breaches Scheme.
6. Complaints Regarding Your Privacy
If you are concerned about how your personal information is handled, you may submit a Move Healthcare Notification Form outlining your complaint. All complaints are managed appropriately and confidentially.
