Introduction

Annie faced significant challenges following multiple surgeries on her right hip. After spending four months in hospital, she was discharged to a Residential Aged Care facility. During her recovery, Annie couldn’t put weight on her right leg for three months and experienced high levels of anxiety and cognitive challenges due to her long-standing mental health issues.

Initial Condition

Two months into her stay at the Residential Aged Care facility, Annie was referred to Move Healthcare. At this time, she was unable to bear weight and relied on a hoist to move in and out of bed, having not walked for five months. Her reluctance to participate in rehabilitation and the facility’s limitations in handling complex cases left her bedbound, with little progress in her mobility.

Personalised Rehabilitation Plan

Recognising Annie’s unique needs, her Move Healthcare physiotherapist focused on building rapport and gradually introducing basic rehabilitation exercises. The initial goal was to boost Annie’s confidence in moving her body and limbs without pain and to help her feel comfortable using her manual handling equipment. With growing trust, Annie transitioned from simple exercises to using a specialised walking frame with the assistance of her physiotherapist. Collaboration with Annie’s aged care carers, her family, and the physiotherapy team was key in her progress.

Achievements and Ongoing Goals

After two months of dedicated rehabilitation, Annie achieved a significant milestone: she was able to get out of bed and sit in a chair without mechanical assistance. This achievement greatly improved her quality of life, allowing her to participate more actively in daily activities and making the caregiving process much easier for her carers.

Annie’s journey did not stop there. She continued her rehabilitation for another three months, demonstrating remarkable dedication and perseverance. Eventually, she was able to walk 50 metres with the support of her physiotherapist and a walking frame. Each step marked a significant victory for Annie and highlighted the effectiveness of her personalised rehabilitation plan.

Annie is now focusing on new challenges, such as walking with her family and carers. These goals are not just about physical improvement but also about enjoying the simple pleasures of life and spending quality time with loved ones. Her progress is a testament to her hard work and the comprehensive support she receives from Move Healthcare.)

Chloe has completed a degree in Sport Rehab and Athletic Therapy as well as a masters in Physiotherapy from the University of Essex in the UK. Chloe has gained experience across a number of areas including musculoskeletal, sport, accident and emergency, stroke, and medical.

Originally from Ireland, Chloe is excited to explore Sydney and experience all that life in Australia has to offer. In her down time she enjoys working out at the gym and going to the beach. 

“My goal as a community physiotherapist is to help people maximise their physical potential in the comfort of their own environment. I strive to give each client the confidence to allow them to continue doing the things they love.”

We’re really excited to welcome Tessa to our team! Here is a little bit about Tessa:

Tessa graduated as a physio from the University of South Australia in 2016. She has worked in a variety of sectors including Aged Care, Disability and private practice.  She has a strong interest in post-illness rehabilitation, and also enjoys providing assessment and training in manual handling and falls prevention in the elderly.

Growing up in the multicultural city of Kuala Lumpur, Malaysia, she also learned to speak Bahasa Melayu, Mandarin, and Cantonese.  In her free time, Tessa enjoys going for outdoor runs, playing badminton, practicing yoga and learning to play the guitar and bass (thanks YouTube!)

The Health and Wellbeing of our clients and staff is of the upmost importance to us at Move Healthcare. We have recently updated out Covid-19 Home Care Safe Work instructions and have also added some extra measures to ensure we are maximising the safety precautions for our clients and staff.
These extra precautions include our staff participating in weekly Covid 19 surveillance screening. This involves our staff taking 2 self-administered rapid Antigen tests each week. Test results will be saved and documented. If a positive result is returned the staff member will take a nose/throat PCR test immediately and isolate as appropriate until results are returned.
We will also ensure our staff, if deemed to be a casual contact and return a negative PCR test, to undertake daily rapid antigen tests for a further 7 days following a negative PCR test.
Both of the above measures go above what is recommended by the NSW Public Health orders.
